As the founder of Unbroken Rehab and Athletics, Dr. Fred Sampang has a unique understanding of the emotional and physical struggles that come with injuries and the desire to return to the activities you love. His own experience of sports injuries during high school and college, and the help of physical therapists, allowed him to recover quickly and return to the game he loved. With his passion for physical therapy, desire to help others avoid the emotional pain of missing out, Dr. Fred believes he found his calling.
Dr. Fred graduated from the University of Illinois-Chicago with a Bachelors in Biological Sciences and Doctorate of Physical Therapy. He then completed a rigorous year of Orthopedic Residency at Houston Methodist Hospital in Sugar Land, followed by another year of fellowship training to specialize in treating athletes. Dr. Fred is not only dedicated to his patients, he also enjoys spending time with his family, training hard 5-6x/week in Crossfit, Oly, and powerlifting, watching and talking about sports, and to unwind he likes to watch stand-up comedy and starting a new series on Netflix.

“Hello! My name is Kimberlyn and I am beyond excited to join the Unbroken team.
While I might be new to the physical therapy profession, I consider myself a PT veteran. Sports were always a passion of mine growing up. I played competitive basketball through highschool until I eventually found Track & Field.
In college, I fell in love with the hammer throw (the most bad-a** field event). After persistent low back and shoulder injury/rehab, I sustained an avoidable career-ending spinal fracture. I was unhappy with the medical attention that I received throughout my collegiate career. So, I decided that I wanted to be better.
My goal as a physical therapist is to educate athletes and provide them with the appropriate tools to facilitate their recovery, to maximize their physical gains, and to guide further independent growth towards healthy relationships with movement.
Physical therapy isn’t scary and it sure-as-heck does not mean that your athletic career is over. It is an opportunity to come back stronger and smarter than you were before.
I am excited to be a part of this team, but I am most of all grateful to have the opportunity to improve current sports medicine health care so that all the work that YOU put into your bodies is valued and respected. I can’t wait to watch you succeed!”


Clinic Manager – West Loop
“Hello! My name is Tim, my pronouns are he/him, and I am so excited to join Unbroken Rehab and Athletics. I moved to Chicago several years ago from Philadelphia.
I got into physical therapy after playing sports all my life and having several injuries. Growing up my sport was lacrosse and played throughout college and worked as a coach at the college level while in PT school at Kean University. I am passionate to getting athletes back into their sport as well as keeping the weekend warrior active or helping a parent to continue to play with their children. I have been fortunate to treat individuals at all levels from youth sports to professional athletes and enjoy creating specific individualized plans to keep you doing what you love.
When I am not in the clinic I will be in the gym or training for endurance races like triathlons or marathons. In my off time I also enjoy cooking, cheering on the Eagles, exploring my new home and trying to teach myself how to play the guitar.”
Physical Therapist – Lakeview
”Hi! My name is Eva (pronounced ay-vuh) and I am really excited to join the Unbroken team!
A little bit about me: I was born and raised in Chicago where I attended Lane Tech High School and played Division I softball at Northwestern University. In playing softball for 14 years, I gained a vast knowledge of sport-specific functional movements as well as barbell lifts and strength and conditioning principles. I fell in love with barbell movements and Olympic lifting and have continued to work on this craft as an athlete post-college. During my high school and college athletic careers, I had my fair share of injuries — most notably a low back injury in college that forced me to sit out of the game I loved.
Looking back, I wished I had rehabilitative care that combated movement-related fear-avoidance and loaded me properly, preparing for the demands of my sport and enabling me to reach my full potential. This is part of the reason why I wanted to pursue a doctorate in physical therapy.
My goal as a physical therapist is to empower and educate athletes on ways they can continue to move through their injury, maximize their performance, and become confident in their abilities again.
While I am not in the clinic or at the gym, I am probably going on adventures like vintage shopping, trying good eats/drinks and hiking.”


Physical Therapist – Park Ridge
“Hello, my name is Will and I am ecstatic to join the Unbroken Rehab and Athletics team!
I have been, and still am, a life-long athlete. Thus, I realize all too well the impact injuries can have on individuals’ chosen athletic endeavors, their mental health, and simply completing daily tasks. I plan to facilitate evidence-based, patient-centered, holistic rehabilitation with the ultimate goal of returning individuals to their pre-injury, active routines in better shape than they were before.
My decision to become a PT was due to positive rehab experiences I had after injuries growing up as well as having an older brother who has been a PT for over 5 years. In fact, my first year in undergrad was my brother’s last year in PT school (at the same university) so I was able to learn what PT was all about.
I’m originally from West Michigan and attended Central Michigan University for undergrad prior to attending PT school at University of Illinois at Chicago. I grew up trying every sport my hometown had to offer and ended up competing in powerlifting, alongside high school sports, all the way through undergrad. In the 4 years since undergrad my athletic focus has shifted to jiu jitsu, with many competitions under my belt and more planned.
Outside of athletics, my free time is spent with my girlfriend and two cats (Nova and Luna) in Elk Grove Village. I enjoy reading a variety of books, catching up on the latest movies/shows, spending time in nature, and venturing to new restaurants in my spare time. I’m a huge foodie who loves all types of cuisines, so feel free to share places to try!”
Physical Therapist – Wheeling, Park Ridge
”I am originally from metro Detroit and became interested in physical therapy after the positive impact it had on me growing up and as a life long athlete. I understand the difficulties, frustrations, and challenges an injury comes with but I believe in the power of PT to help overcome a set back. I graduated with my Bachelor’s degree in Exercise Science from Eastern Michigan University and later earned my Doctorate of Physical Therapy from Western Kentucky University.
My clinical interests include post-operative care, return to sport rehabilitation, return to performance, and general orthopedic conditions. I am dedicated to providing patients individualized evidence-based care to educate, empower, maximize recovery, and restore an athlete’s ability to get back to what they love.
In my free time I enjoy spending time with my wife and our dog Ivy, hiking, running, golfing, playing basketball and watching Detroit sports.”


Physical Therapist Assistant – West Loop
“Hi, I’m Jerick! I’m excited to be part of the Unbroken Rehab and Athletics team.
I wasn’t the most athletic growing up, but discovering CrossFit early in my career completely changed the way I view health and fitness. It sparked my passion for movement, strength, and helping others feel their best. While I haven’t experienced a major injury myself, I deeply understand the challenges of recovery and the patience it requires.
With several years of experience, I’m passionate about providing high-quality care through a fitness-forward approach. My focus is on helping people recover from injuries and confidently return to the sports, hobbies, and activities they love. I’m committed to continually learning and growing so I can deliver the best possible care to every patient.
Outside the clinic, I enjoy spending time with my wife and our little son—usually with a good coffee and maybe a sweet treat—or training at the gym to keep getting fitter every day. I also love traveling and exploring new cities when I get the chance.”

Immanuel “Manny” Carr is a Chicagoland native who moved around from Southeast Chicago, Plainfield, Hinsdale and now Rogers Park.
He grew up playing sports at a young age starting with soccer, then football at the age of 10, up until a year of Division III football and some wrestling in Highschool as well.
At the age of 14, weight training was introduced to his life and ever since then it has remained a consistent part of it.
He is now a certified USAW-L1, who also has a passion for yoga/meditation, being fit in all phases of life: mind, body, and spirit is a main focus.
He look forward to being part of the team, and meeting the members of the Unbroken community
