KIMBERLYN GADAWSKI

Director of Operations

“Hello! My name is Kimberlyn and I am beyond excited to join the Unbroken team.

While I might be new to the physical therapy profession, I consider myself a PT veteran. Sports were always a passion of mine growing up. I played competitive basketball through highschool until I eventually found Track & Field.

In college, I fell in love with the hammer throw (the most bad-a** field event). After persistent low back and shoulder injury/rehab, I sustained an avoidable career-ending spinal fracture. I was unhappy with the medical attention that I received throughout my collegiate career. So, I decided that I wanted to be better.

My goal as a physical therapist is to educate athletes and provide them with the appropriate tools to facilitate their recovery, to maximize their physical gains, and to guide further independent growth towards healthy relationships with movement.

Physical therapy isn’t scary and it sure-as-heck does not mean that your athletic career is over. It is an opportunity to come back stronger and smarter than you were before.

I am excited to be a part of this team, but I am most of all grateful to have the opportunity to improve current sports medicine health care so that all the work that YOU put into your bodies is valued and respected. I can’t wait to watch you succeed!”

TIMOTHY FLOUNDERS

Clinic Manager – West Loop

Hello! My name is Tim, my pronouns are he/him, and I am so excited to join Unbroken Rehab and Athletics. I moved to Chicago several years ago from Philadelphia.

I got into physical therapy after playing sports all my life and having several injuries. Growing up my sport was lacrosse and played throughout college and worked as a coach at the college level while in PT school at Kean University. I am passionate to getting athletes back into their sport as well as keeping the weekend warrior active or helping a parent to continue to play with their children. I have been fortunate to treat individuals at all levels from youth sports to professional athletes and enjoy creating specific individualized plans to keep you doing what you love.

When I am not in the clinic I will be in the gym or training for endurance races like triathlons or marathons. In my off time I also enjoy cooking, cheering on the Eagles, exploring my new home and trying to teach myself how to play the guitar.”

EVA STEES

Physical Therapist – Lakeview

”Hi! My name is Eva (pronounced ay-vuh) and I am really excited to join the Unbroken team!

A little bit about me: I was born and raised in Chicago where I attended Lane Tech High School and played Division I softball at Northwestern University. In playing softball for 14 years, I gained a vast knowledge of sport-specific functional movements as well as barbell lifts and strength and conditioning principles. I fell in love with barbell movements and Olympic lifting and have continued to work on this craft as an athlete post-college. During my high school and college athletic careers, I had my fair share of injuries — most notably a low back injury in college that forced me to sit out of the game I loved.

Looking back, I wished I had rehabilitative care that combated movement-related fear-avoidance and loaded me properly, preparing for the demands of my sport and enabling me to reach my full potential. This is part of the reason why I wanted to pursue a doctorate in physical therapy.

My goal as a physical therapist is to empower and educate athletes on ways they can continue to move through their injury, maximize their performance, and become confident in their abilities again.

While I am not in the clinic or at the gym, I am probably going on adventures like vintage shopping, trying good eats/drinks and hiking.”

WILL GATES

Physical Therapist – Park Ridge

“Hello, my name is Will and I am ecstatic to join the Unbroken Rehab and Athletics team!

I have been, and still am, a life-long athlete. Thus, I realize all too well the impact injuries can have on individuals’ chosen athletic endeavors, their mental health, and simply completing daily tasks. I plan to facilitate evidence-based, patient-centered, holistic rehabilitation with the ultimate goal of returning individuals to their pre-injury, active routines in better shape than they were before.

My decision to become a PT was due to positive rehab experiences I had after injuries growing up as well as having an older brother who has been a PT for over 5 years. In fact, my first year in undergrad was my brother’s last year in PT school (at the same university) so I was able to learn what PT was all about.

I’m originally from West Michigan and attended Central Michigan University for undergrad prior to attending PT school at University of Illinois at Chicago. I grew up trying every sport my hometown had to offer and ended up competing in powerlifting, alongside high school sports, all the way through undergrad. In the 4 years since undergrad my athletic focus has shifted to jiu jitsu, with many competitions under my belt and more planned.

Outside of athletics, my free time is spent with my girlfriend and two cats (Nova and Luna) in Elk Grove Village. I enjoy reading a variety of books, catching up on the latest movies/shows, spending time in nature, and venturing to new restaurants in my spare time. I’m a huge foodie who loves all types of cuisines, so feel free to share places to try!”

MARTIN URBANIAK

Physical Therapist – Wheeling, Park Ridge

”I am originally from metro Detroit and became interested in physical therapy after the positive impact it had on me growing up and as a life long athlete. I understand the difficulties, frustrations, and challenges an injury comes with but I believe in the power of PT to help overcome a set back. I graduated with my Bachelor’s degree in Exercise Science from Eastern Michigan University and later earned my Doctorate of Physical Therapy from Western Kentucky University.

My clinical interests include post-operative care, return to sport rehabilitation, return to performance, and general orthopedic conditions. I am dedicated to providing patients individualized evidence-based care to educate, empower, maximize recovery, and restore an athlete’s ability to get back to what they love.

In my free time I enjoy spending time with my wife and our dog Ivy, hiking, running, golfing, playing basketball and watching Detroit sports.”

JERICK ISLAN, PTA

Physical Therapist Assistant – West Loop

“Hi, I’m Jerick! I’m excited to be part of the Unbroken Rehab and Athletics team.

I wasn’t the most athletic growing up, but discovering CrossFit early in my career completely changed the way I view health and fitness. It sparked my passion for movement, strength, and helping others feel their best. While I haven’t experienced a major injury myself, I deeply understand the challenges of recovery and the patience it requires.

With several years of experience, I’m passionate about providing high-quality care through a fitness-forward approach. My focus is on helping people recover from injuries and confidently return to the sports, hobbies, and activities they love. I’m committed to continually learning and growing so I can deliver the best possible care to every patient.

Outside the clinic, I enjoy spending time with my wife and our little son—usually with a good coffee and maybe a sweet treat—or training at the gym to keep getting fitter every day. I also love traveling and exploring new cities when I get the chance.”

I have used Unbroken twice this year for sports related injuries and they have been invaluable for my recovery and continued performance after our sessions. Dr. Kimberlyn and the team do an amazing job of assessing your injury and your body as a whole in order to properly address the problem and start the healing process. They have a wealth of tools and techniques including dry needling, cupping and compression boots that make the road to recovery quick and easy.

I came to Unbroken Rehab and Athletics after popping my gastrocnemius while training for Hyrox last year. I met the Unbroken team at our gym after they did an event talking about injury prevention. I called them the same day of my injury and they got me in right after seeing a doc hours later. They got me back to walking, then running within 3 months allowing me to compete just in time. Unbroken Rehab is the spot for all athletes, anyone who trains, competes or likes to move.

I highly recommend them to anyone who needs support getting back to feeling good and back to what they love. Recently, they’ve been treating my arm that was injured during wall balls.I am deeply appreciative of their unwavering support and profound expertise in the realm of athletics. Thanks, Unbroken!

After dealing with shoulder pain for months, I finally made a call to Unbroken Rehab. I was connected with Fred and went to see him based on strong recommendations—and I’m so glad I did. Not only did he help me become completely pain-free, but he also took the time to educate me on why I was getting hurt and how to correct my movements to prevent it from happening again. As someone who does CrossFit, I really appreciated how knowledgeable he is about the sport and how he tailored my treatment to fit the demands of my workouts. Fred is professional, attentive, and genuinely cares about helping his patients move and feel their best. Im so happy I finally made the call, Unbroken Rehab was the best decision for my road to recovery and getting me back to doing what I love.

Whenever someone needs a recommendation for a physical therapist, I always recommend the Unbroken Team. Fred and his staff are so incredibly knowledgeable, and they treat each patient like an individual, not just a number. The road to recovery is difficult and the team is there with you every step of the way. I cannot say enough good things about them, their work truly speaks for itself. It has been a pleasure to work with Will over the last four months. I will always be grateful for the work they did to help me become unbroken 🙂

I was struggling to do a variety of exercises at the gym, to the point where I stopped doing them altogether because of the pain and discomfort. Dr. Martin Urbaniak helped me resolve the issue, and after a few visits and following his treatment plan, I’m now able to do the exercises I had previously given up on.

I highly recommend Unbroken Rehab and Athletics, as well as Dr. Martin Urbaniak, if you’re dealing with issues that are keeping you from returning to your regular activities.

I had a great experience working with the crew at Unbroken Rehab. Worked with Tim, Eva, and Jerick who were all very passionate about getting me back into my circus sport. I came in because of shoulder pain that prevented me from training. They did an in-depth evaluation and felt seen and heard when they listened. I appreciated the personalized plan that helped strengthen the muscles required for my activity. They were very knowledgeable and answered any questions I had.

Huge pro is the 1:1 time, which is difficult to find now a days. I’ve been to previous practices where I saw a new person each time so it was difficult receiving continuity of care and see results. At Unbroken Rehab I left with more awareness of my body and feel more confident returning back to my sport. If you’re an athlete experiencing an injury or pain, definitely recommend checking them out!

I cannot say enough great things about Fred and his team! Fred is knowledgeable, patient and a supportive physical therapist! My doctor was pushing for surgery but after working with Fred that option was out the window! The sessions were not only effective but also tailored to my specific needs. I noticed real improvements in my mobility and pain levels, and thankfully now I am back to work, feeling great again! I highly recommend working with Fred! A five star experience!

I reached out to Kimberlyn after years of lower back issues from a combination of work and lifting. Kimberlyn was exceptionally knowledgeable and while working with her I noticed significant improvements within a couple months to the point where its no longer been an issue. She has been extremely knowledgeable for any questions about general physical wellness helped me craft a warmup routine through the various stretches she provided to keep any further lower back problems at bay. I’ve since returned for a neck injury which I sustained at work which healed quickly which I believe I owe a lot of it to Kimberlyn for physical therapy. I feel at this point I’m injury prone and for that I’m glad that Unbroken is available whenever I require assistance with recovery.
Response from the owner 8 months ago

Thanks for the review Andrew! You have demanding a job and hobbies that take a toll on your body, so don’t feel that you are injury prone. Regardless we will always be in your corner should you need our assistance in rehabbing again in the future!

I highly recommend Unbroken for any and all physical therapy needs. I worked with Kimberlyn following shoulder surgery and had a fantastic experience. She is knowledgeable, fun, and puts her clients first. I really appreciated that her approach included working on movements/activities that were important to me and my career. The facility is top notch – clean, reputable equipment, and plenty of space. One of the best things about Unbroken is 1-on-1 time with a DPT – a perk I am unaware of anywhere else offering. This is a top tier facility with top tier staff. Look no further, go with Unbroken!

Fred is an exceptional physical therapist at Unbroken Rehab. From day one, he created a space where I felt both supported and challenged. I came in scared and unsure about how my wrist would heal, but Fred immediately helped me understand what was really going on. What I appreciated most is that he didn’t let me stay stuck in fear—he encouraged me to trust the process and gently pushed me past my comfort zone. Thanks to his guidance, my wrist has improved drastically, and I feel like I’ve gained both strength and knowledge. I’m so grateful to have worked with him and highly recommend Unbroken Rehab for anyone looking for expert, compassionate care.

My weekly sessions at Unbroken Rehab made all the difference in the world with my recovery and continued training. Fred and the Unbroken Rehab team are top notch! Unlike the corporate physical therapy shops, Unbroken Rehab creates highly customized, individualized treatment plans along with closely supervised therapy sessions. On top of that, Fred is a lot of fun and keeps sessions lively yet focused. I hope I won’t have a reason to go back but the next time I’m injured, Fred and Unbroken Rehab will be my first call.

Fred came highly recommended by a fitness trainer at my gym and I can see why- he is an incredible physical therapist! I came in for an elbow injury that I had no idea how I got, and we worked towards a goal on building up my upper body strength which in turn resolved my injury quicker than I expected. He is very easy to talk to, extremely knowledgeable, and truly has the best intentions for all his patients.

I came to Fred and Unbroken Rehab desperate for help with a shoulder injury that I had sustained two years ago that progressively worsened to the point that nearly every overhead movement I performed was causing significant pain. My sport of choice (Olympic weightlifting) demands overhead strength and stability and my shoulder limitations left me unable to train properly and compete.

Fred was able to get me into his clinic (which is impeccably clean and organized) quickly and informed me that my issues could be remedied without surgery(!). Over the course of two months, I was given exercises specifically tailored to my needs that dramatically improved my flexibility and overhead strength, all but eliminating the pain that I had been experiencing for months on end.

To anyone looking to not only reduce discomfort but more importantly get back to their pre-injury level of performance, I cannot recommend Unbroken Rehab enough. Thank you, Fred!

I spent a couple months working with Fred and Amanda after a skiing injury. Fred identified a treatment quickly and made planning and scheduling easy. I worked with Amanda for most of the sessions and she was great to work with. I learned a lot and found some great exercises to improve my core and ensure I can continue to be active. I highly recommend working with Unbroken rehab.
